Come qualsiasi altro intervento chirurgico, anche gli impianti dentali comportano dei rischi. In questo post vi parlerò del rischio più grande che sicuramente vorreste evitare ad ogni costo. Ora vi mancano pochi minuti per acquisire le abilità per identificare ed evitare questo rischio per voi.
Sto scrivendo questo articolo nel tentativo di rispondere alle molte persone con cui abbiamo discusso ultimamente che affermano come altre cliniche (per lo più in Albania) siano “più economiche” e su questa base ignorano completamente i rischi connessi. Una cosa che voglio chiarire in questo articolo è che raramente c’è un grande vantaggio in termini di costi per gli impianti dentali a meno che non si faccia un compromesso con i benefici a lungo termine per la salute del paziente, e questo è in realtà il rischio più grande per voi.
Non fraintendetemi – non tutte le cliniche in Albania/ Bulgaria/ Croazia/ Germania/ Regno Unito/ …ovunque seguono protocolli sbagliati e non tutti seguono quelli buoni – ci sono “cattivi ragazzi” ovunque: in Bulgaria, in Albania e nel Regno Unito , ecc. E il contrario: ci sono “bravi ragazzi” ovunque. Il trucco per evitare il rischio di essere truffati è imparare a riconoscerli.
La tentazione di dire “SI”
Immaginate (!) di gestire una nuova clinica dentale con tutti i suoi investimenti e spese. Oggi avete una richiesta da una potenziale paziente del Regno Unito, chiamata Anna in questa storia immaginaria per impianti dentali. Anna vuole un preventivo. Ovviamente volete sembrare professionali e richiedete la radiografia di Annа (anche se non significa quasi nulla nel protocollo per gli impianti!) e lei ve ne invia una che dimostra le sue cattive condizioni dei denti. Ora, ecco il momento difficile per voi.
Il vostro dilemma morale: sceglierete l’onestà e la morale oppure andrete а ricercare il denaro! È così, mi dispiace doverlo ripetere, ma una radiografia NON SERVE QUASI A NIENTE per poter provare la sufficienza di osso per gli impianti, e senza osso significa nessun impianto. Almeno così dovrebbe essere nel mondo ideale…
Ovviamente sapete che Anna ha chiesto informazioni ad almeno altre 5 cliniche e dovete sbrigarvi. Immaginiamo che siete pressati dalle vostre spese aziendali e decidete di ignorare ciò che avete imparato all’università di odontoiatria. Avete bisogno di Anna e dei suoi potenziali 5000+ Euro/GBP. Dovete convincere Anna: le mandate un’offerta di prezzo ALL-ON-6 incredibilmente basso per entrambe le arcate, un’offerta che raramente può essere superata dagli altri. In fondo nella vostra mente sperate solo che Anna abbia abbastanza osso…
E ora arriva il rischio per Anna! Si innamora della vostra bella voce al telefono e accetta la vostra offerta con volo, alberghi, trasferimenti, cibo, gite gratuiti e qualsiasi altra cosa gratis. Anna arriva in clinica confidando nei medici e nel loro giuramento di Ippocrate. Lei non sa nulla dei protocolli per gli impianti dentali (il suo errore!). Arriva e incontra la versione più simpatica di voi, e andando avanti si scopre (tramite una TAC 3D) che Anna non ha abbastanza osso (in circa il 50% dei casi) – quindi non è adatta per gli impianti ! Beh…, sfortuna… cosa decidete?
–dire la verità ad Anna, deluderla molto e pagarle il volo, l’albergo, tutto il resto e farla arrabbiare??
– o nascondere i fatti, andare avanti e correre il rischio ingiustificato di inserire gli impianti, che potrebbero portare a infezioni e grandi complicazioni future??
Ricordate, sapevate bene prima che Anna venisse nella vostra clinica che poteva non essere adatta per gli impianti e tuttavia le avete inviato quella “grande” offerta ALL-ON-4/6. Sapevate a cosa andavate incontro… e ora devete finire ciò che è stato iniziato e decidete di posizionare questi impianti. Dopo poche ore Anna viene felicemente impiantata e dopo un giorno o due Anna vede i suoi nuovi denti. Sorridete alle foto e sperate solo che le complicazioni e i problemi non tornino abbastanza presto… Credetemi, non vorreste essere al posto di Anna.
Un caso reale!
Ora torniamo alla vita reale… Ai fini di questo articolo ho contattato alcune cliniche casuali che pubblicizzavano su Facebook. Belle pubblicità tra l’altro… alcune erano anche davvero ECONOMICHE. Tre su quattro hanno detto quel maledetto “SI”! Ecco come è andata la mia conversazione (vedi screenshot qui sotto):
Avete notato il “SI”??!! Mi hanno offerto un ALL-ON-6 (una costruzione fissa su 6 impianti) per entrambe le arcate. Senza esitazioni e dubbi! Perfetto, fantastico, mio padre finalmente avrà di nuovo i denti… o forse non per molto… a meno che questa clinica non abbia fatto scoperte innovative nell’implantologia…
Durante la telefonata, la simpatica signora dall’altra parte del telefono mi ha avvertito che non offrono corone in zirconio su impianti perchè “non vanno mai fatte e se altre cliniche me le offrono mi stanno truffando”. (Cosa?!!) Inoltre, il mio secondo viaggio dovrebbe essere minimo di 10 giorni, il che mi porta a pensare che non hanno un laboratorio interno e si affidano ad uno esterno che non è mai una cosa buona. Ma tutto questo è di secondaria importanza. L’importante è che hanno deciso di inseguire il denaro senza preoccuparsi della mia salute.
Ecco un altro esempio (clicca qui per vedere lo screenshot). Abbastanza divertente il fatto che questa volta con la stessa radiografia ho ricevuto una “diagnosi” diversa, cioè che i denti devono essere estratti e che verranno posizionati 8 impianti subito dopo l’estrazione (wow!), che di solito va contro il protocollo di implatologia. Questo dimostra solo ciò di cui ho parlato in precedenza: la radiografia panoramica non è la base per pianificare gli impianti dentali e le cliniche solitamente sono tentate dalla parte economica.
Inoltre, molto spesso le cliniche tendono a parlare della marca degli impianti piuttosto che della marca dello zirconio (il materiale più comune nei restauri dentali). Lo zirconio economico può essere 10 volte più economico dello zirconio di qualità (clicca qui per vedere i dettagli). A volte è necessaria una barra in titanio, che aumenta ancora di più il prezzo. D’altra parte, lo zirconio, sebbene popolare, non è sempre il miglior materiale per All-on-4/6 perché è più pesante e meno flessibile come il G-CAM, ad esempio. Quello che voglio dire è che c’è molto di più da considerare oltre al semplice “All-on-4”, ma molte cliniche preferiscono tacere su questo. Sfortunatamente, molto spesso il cliente-paziente abituale non è a conoscenza di tutto questo e diventa una facile vittima.
Il fatto economico è che le cliniche che lavorano con margini di profitto bassi avranno maggiori difficoltà a dire “NO” (cioè dare cattive notizie) al potenziale paziente. Non tutti lo fanno, non fraintendetemi, dico solo che la possibilità del “SÌ” è più alta! Tali cliniche sono pressate da ricavi inferiori e ogni paziente nuovo è importante per mantenere un flusso di entrate costante. Un’impresa nel campo medico richiede ingenti investimenti, manodopera qualificata costosa, grandi budget per il marketing e team di vendita. Trovare un equilibrio tra tutto questo è la chiave per sopravvivere in questo business.
Non abbiate dubbi sul fatto che un’attenta analisi di una TAC 3D è l’unico esame necessario per confermare un caso per l’implantologia. La TAC 3D è molto obiettiva: o avete abbastanza osso oppure no. Naturalmente in alcuni casi è possibile eseguire l’innesto osseo, ma in questo caso raramente vengono date delle garanzie. (Clicca qui per saperne di più). Nel 2012 l’Accademia americana di radiologia orale e maxillofacciale (AAOMR) ha rilasciato una dichiarazione ufficiale “che la TAC 3D (CBCT) è il metodo di immagini migliore da scegliere per ottenere la valutazione di tutti i siti implantari dentali” dopo aver valutato tutti i metodi radiologici utilizzati nel trattamento dentale (ecco il riferimento originale).
Nel nostro caso, al Sofia Implant Centre sentiresti il nostro consulente dire che avremo bisogno di una TAC 3D prima di confermare il vostro caso. Siamo sempre alla ricerca di un buon equilibrio tra il prezzo e ciò che offriamo. Non siamo né i più economici, né vogliamo esserlo e non abbiamo neppure la necessità di esserlo. Il prezzo per noi è un effetto collaterale dei nostri sforzi per fornire un buon rapporto qualità-prezzo. Il nostro messaggio per il paziente è sempre uno: il vostro beneficio per la salute a lungo termine è alla base di tutte le nostre decisioni e azioni. A volte questo crea attrito: dare cattive notizie non è mai facile. Continueremo a dire “NO” nei casi senza giustificazione medica. La nostra promessa è che non metteremo mai, mai, MAI! un impianto in caso di mancanza di osso a sufficienza. Per noi questa è la strada giusta da percorrere.
Cosa ricordare
La cosa più importante da ricordare è che nessuna clinica/medico può dirvi onestamente che può posizionare degli impianti finché non analizzerà la vostra TAC 3D. Se lo fanno, state alla larga, non importa quanto sia economica la loro offerta. Cercare semplicemente la più “economica”, probabilmente tra un anno o due vi verrà a costare molto di più, non solo economicamente, ma anche in termini di tempo e sofferenza. Non seguire il giusto protocollo per l’implantologia è il rischio più grande che potreste correre. Dovete sapere come riconoscerlo.”Economico” aumenta solo i rischi.

I had full upper and lower all on six implant treatment at the Sofia Implant Centre in February 2026, followed by permanent zirconium bridges in August. The results are exceptional — my smile looks fantastic and I can eat normally again. My thanks to Dr Rafid Hussain, the head surgeon, for the outstanding surgical work, and to Dr Ogur Inal, who fitted my zirconium bridges with real precision and care. I recently visited Sofia Implant Centre for a check-up of my All-on-4 implants, which I had completed there 3 years ago, and my experience continues to reward me for choosing their excellent services. My consultant this time, Diyan Shkodrov, was extremely helpful with booking the hotel and arranging my transfer, along with the sweetest clinic guide at the airport, whose name I unfortunately forgot. My meeting during the check-up with dental specialist Dr Kliment Bekiarov, who was also responsible for creating my final smile, was as always very warm, welcoming, and full of positive energy. He was more than willing to answer all my questions, and the check-up showed that everything was flawless. Sofia Implant Centre is one of the most modern and top-quality dental clinics, adhering to the highest European standards with excellent equipment, impeccable cleanliness, and outstanding organised patient care. Everyone on staff—from the drivers and receptionists to the nurses, consultants, and doctors—is dedicated and highly specialised in their role. The system works seamlessly, inspiring trust, organisation, and professionalism. I greatly appreciate the doctors' honesty and ethics, which clearly outweigh short-term profit, as well as their detailed transparency. This was evident in the meeting following the panoramic X-ray to determine the treatment plan. My case was particularly challenging for All-on-4 in the upper jaw due to my very thin bone structure, which carried the risk that the implants might not integrate after the multiple extractions required. The decision was ultimately mine on whether to proceed. My consultant at the time, Lina—a Greek woman raised in Bulgaria—was always willing to explain, advise, and support me on a personal level throughout our communication, from start to finish. So, I decided to take the leap, go ahead with the extractions (which had to be done eventually anyway), and move forward with the implants. The surgery was a success, and everything went perfectly thanks to the surgeon and the nurse assistant. It was a long and thorough procedure that required delicate handling, expertise, and, of course, patience from all of us. My bone was thin but dense and strong. I mention this because while an X-ray may show available bone, it cannot always reveal its actual quality or density. The follow-up process was very satisfying, both for my temporary teeth and especially for the final set, where we used Digital Smile Design simulations. Dr Kliment Bekiarov carefully listened to and discussed all my preferences regarding the shape, size, and colour of my teeth. As a result, I now have a wonderful, natural-looking smile that feels completely my own. They are fully functional, and the total cost was significantly lower than it would have been in Greece. The clinic is fabulous and you're well cared for throughout the whole process and I would highly recommend Sofia Implant Centre. They arrange airport pick up, provide details of nearby hotel accommodation and free clinic driver to collect you from your hotel to the clinic and back for all appointments. The only reason I have given 4 stars is because despite the colour of my implants being lightened, I'm still not happy with the colour and could do nothing about it as it was my final fitting.

From early stages inquiry Kris talked me through the process booked my flights and Sofia implant centre did the rest got to choose my hotel which this you pay yourself but get a discount via the clinic. All the transfers to and from the clinic was taken care of. All the staff were professional and welcoming went over my plan after 3d cray everything was explained and the costs were broken down. All medication was provided pain, antibiotics and vitamin c all to help with the healing process. At the end of a week of treatment I was fitted with my temporary teeth after agreeing color and size for 6 months then back for my permanent. Thank you to all the team from the drivers to the surgeons and everyone else like receptionists and nurses. The centre is kept immaculately clean and got shown round the centre even the lab in house that make the teeth. Definitely recommended it.

my journey at Sofia implant started in 2024 , I had a negligent dentist in the UK and ended up with severe gum disease . I contacted various dentists abroad Sofia implant center was the only one who was interested in saving as many of my own teeth as possible .I spoke to Kris who co ordinated everything from start to Finnish including booking hotels arranging for me to be picked up from the airport and he was always there if I needed anything .Dr Hussain extracted the bad teeth also he is a maxillofacial surgeon who repaired a scar on my lip for which I am eternally grateful for . I decided to have the guided implants ,Dr Patchev put them in for me , this wasn't pain free but was bearable and everything was explained to me .For the next part I was under Dr Jana who fitted the teeth onto the implants all on 5 top 4 implants on bottom and 6 crowns .All was finished in Dec 25 I am extremely happy with the outcome so a massive big thanks to everyone who treated me the front desk staff and the drivers bobby and angel .Since returning to the UK I have visited their Stockport branch for a little correction , to anyone who is considering implants ,Sophia implant center is second to none. An extra thank you to Kris who went above and beyond . Sofia implant center is a very professional state of the art center ,I would highly recommend them thank you again for my new smile 😁

I had all on 4 on the top,and a full set of crowns on the bottom. This is a super modern,well equipped and staffed dental hospital,where every part of your treatment is done by a specialist. I have just had my first stage of treatment completed (crowns) as well as root canal treatments and fillings. I am very impressed with the service I have received so far.
